FOR AGED CARE PROVIDERS
Lifestyle programs that residents look forward to.
LIFE+ can supplement a facility’s lifestyle calendar with scheduled sessions, outings and one-to-one experiences, delivered by AYS staff and reported back to the lifestyle team.
Program plan aligned to residents’ interests
Attendance and engagement notes after each session
Regular review with lifestyle and care staff
FOR PEOPLE WITH DISABILITY
Everyday skills, real community places.
Programs such as Skills+, Connect+ and My Day+ build confidence in shopping, transport, social settings and routines — in the places people actually live their lives.
Goals are set with the participant and their supporters, and progress is reviewed together.
